AAHFN

OnTrack to Health is proud to partner with AAHFN to co-develop educational content for heart failure patients. Educational content is delivered directly to patients through their use of the OnTrack to Health mobile application. As patients under the direction of their care team self-manage their heart failure, patient compliance and wellness triggers educational content specific to their current situation. Milner Fenwick

Milner Fenwick HealthClips help create a “just-in-time” learning environment for OnTrack patients. By providing high-quality, relevant educational content we are accelerating the feedback loop to keep the patient ontrack to health. Teach back tools can also be implemented to confirm patient understanding of the learning videos and provide follow-up if necessary. Learn more at: http://www.milner-fenwick.com/

University of Kentucky

OnTrack to Health has developed a collaborative working relationship with the University of Kentucky. We anticipate starting a five-year clinical trial in late 2014. The trial will involve 400 patients, with the focus on testing the efficacy of OnTrack to Health in promoting self-management of chronic diseases in underserved populations. Dr. Terry Lennie, Dr. Debra Moser, and a distinguished team of co-investigators will run the trial. Learn more at: http://www.uky.edu/
